The goal is to ensure timely assessment and intervention as needed. Ideal candidate has strong clinical experience and desire to work with patients of all ages in health maintenance and disease prevention.

- Combination of both nursing home and primary care practice
- Round in nursing homes for well visits and in response to patient needs
